SANITARY PERMIT

 

  1. About The Service:

In accordance to the implementing rules and regulations (IRR) of Presidential Decree 856  otherwise known as the Sanitation Code, all business establishments located within this municipality are obliged to secure a Sanitary Permit under their designated Rural Health Unit. It is the responsibility of the Sanitary Inspector and Municipal Health Officer or Rural Health Physician to entertain applicants, check for completeness and evaluate all the required documents and inspect the establishment prior to the issuance of a Sanitary Permit.

All business establishments operating within the municipality either new applicants or for renewal of permits.

MEDICAL SERVICES

 

  1. About The Service:

Provide medical and other health assistance to citizens.

 

  1. Who Can Avail The Service:
  1. Consultation

Persons with medical problems who need to avail of medical checkups and medicines

  1. Dental Service

Persons with dental problems and need to avail of teeth extractions and repairs

  1. Expanded Program in Immunization (OPV, BCG, DPT, Hep B, TT and Measles)

Mothers with infants who need immunizations with the following vaccines: BCG, DPT, OPV, Hepa B, Anti-Measles and Tetanus toxoid

  1. Availing of Anti-tuberculosis Drug

Persons with diagnosed tuberculosis and need anti-tuberculosis medicines and care

  1. Availing of Anti-leprosy Drugs

Persons with diagnosed leprosy and need anti-leprosy drugs and care.

  1. Availing of Family Planning Services

Couples who need to avail of contraceptive methods either natural or artificial such as pills, DMPA, IUD, BTL and condoms

  1. Pap’s Smear

GRO’s and women who requires pap’s smear in accordance to municipal ordinances or for simple diagnostic procedures

  1. Minor Surgeries (Excision, Circumcision, Skin Repair)

Persons who may require minor surgical procedures available at the health center

  1. Issuance of Medical Certificate

Persons who need a medical certificate as a requirement for job or school applications, medical excuses and other related matters.

  1. Issuance of Death Certificate and Transfer of Cadaver

Persons with relatives who died within the municipality and requires registration of death persons whose deceased relative will be buried outside the municipality

  1. Availing of Referrals to Secondary and Tertiary Hospitals

Persons who require further diagnostics and management to secondary or tertiary hospitals
