- About The Service:
The issuance of registered civil registry documents is pursuant to the provision of Republic Act No. 3753 otherwise known as the “Civil Registry Law”.

- All citizens born/died/married in the municipality
- Nearest kin of the document owner
- Authorized representatives of the document owner

REGISTRATION OF DEATH CERTIFICATE
- About The Service:
It shall be the responsibility of the nearest relative or spouse who has knowledge of the death to report the same within forty-eight (48) hours if the deceased died without medical attendance. The health officer shall examine and sign the death certificate and direct the registration of the death certificate to the Office of the Civil Registrar within the reglamentary period of thirty (30) days.

REGISTRATION OF BIRTH AND MARRIAGE CERTIFICATES
- About The Service:
Republic Act No. 3753 mandates the establishment of a civil register in the Philippines where acts, events, legal instruments and court decrees concerning the civil status of a person shall be recorded.
The birth of the child, being a vital event of a person, shall be registered within thirty (30) days from the time of birth in the office of the Civil Registrar of the city/municipality where the birth occurred.
For ordinary marriage, the time for submission of the Certificate of Marriage is fifteen (15) days following the solemnization of marriage while for marriage exempt from license requirements, the prescribed period is thirty (30) days, at the place where the marriage was solemnized.

REQUESTING ENDORSEMENT OF CIVIL REGISTRY RECORDS TO THE OFFICE OF THE OCRG-NSO
- About The Service:
As a rule, all civil registrars shall submit civil registry documents to the Office of the Civil Registrar General (OCRG) thru their respective NSO provincial offices.
There are instances when the OCRG-NSO cannot issue copy/copies to the interested party because their office have no available record in its archive or the current document is still with the NSO provincial office being processed.
To facilitate the issuance of requested documents, the concerned Provincial Statistics Officer (PSO) or Civil Registrar is required to submit or endorse the needed document on a piecemeal basis to the OCRG-NSO.

DELAYED REGISTRATION OF CIVIL REGISTRY RECORDS (BIRTH, DEATH, MARRIAGE)
- About The Service:
Delayed registration of birth, death and marriage like ordinary registration made at the time of the event shall be filed at the Office of the Civil Registrar of the place where the event occurred, following the lapse of the reglamentary period to register.

APPLYING FOR A MARRIAGE LICENSE
- About The Service:
Where a marriage license is required, each of the contracting parties shall file separate sworn application for such license with the proper local civil registrar of the place where either or both of the contracting parties reside.
The local civil registrar concerned shall enter all applications for marriage license filed with him/her in a registry book strictly in the order in which the same are received.
When the license is issued, the same shall be valid in any part of the Philippines for a period of one hundred twenty (120) days from the date of issue and shall be deemed automatically cancelled at the expiration of said period if the contracting parties have not made use of it.

Male or female resident of the municipality who are eligible to contract marriage.

REGISTRATION OF FOUNDLING/ABANDONED CHILDREN
- About The Service:
A foundling is a deserted or abandoned infant or child found, with parents, guardian or relatives being unknown, or a child committed in an orphanage or charitable or similar institution with unknown facts of birth and parentage.
The finder/charitable institution within thirty (30) days shall make registration of the foundling in the Office of the Civil Registrar of the municipality where the child was found from the date of finding/commitment of the child.
Any report made beyond the reglamentary period of 30 days shall be considered late and the concerned party-registrant shall be required to state in a sworn statement the circumstances that caused the late reporting to the civil registrar.
Immediately after finding a foundling, the finder shall report the case to the barangay captain of the place where the foundling was found, or to the police headquarters whichever is nearer or convenient to the finder. When the report is duly noted, either by the barangay captain or by the police authority, the finder shall commit the child to the care of the Department of Social Welfare and Development or to a duly licensed orphanage or charitable institution or similar institution. Upon commitment, the finder shall give to the charitable institution hi/her copy of the Certificate of foundling, if he has registered the foundling.
In case the finder is awarded the custody of the foundling by the proper authority, he shall give a name for the child and shall report the same to the civil registrar of the municipality where the child was found. Otherwise, the giving of name to the child and its registration as foundling shall be the responsibility of the Department of Social Welfare and Development or the orphanage or charitable or similar institution where the child was committed.

REGISTRATION OF COURT ORDER/DECREES AND REQUEST OF ANNOTATED RECORD
- About The Service:
Like other civil registry documents, court decrees concerning the status of a person shall be registered in the civil registrar’s office where the court is functioning, within ten (10) days after the decree/order has become final and executory.

REGISTRATION OF LEGAL INSTRUMENTS/LEGITIMATION OF NATURAL CHILD
- About The Service:
As a general rule, all legal instruments shall be registered in the civil registry of the place where they were executed except the following:
- Affidavit of reappearance – where the parties to the subsequent marriage are residing.
- Marriage Settlement – where the marriage was recorded.
- Admission of Paternity; and
- Acknowledgement; Legitimation; Voluntary Emancipation of Minor;
Parental Authorization or Ratification of Artificial Insemination – where the birth of the child was recorded.
All legal instrument executed abroad shall be registered in the civil registry office of Manila.
Not falling under the aforementioned exceptions are the following registrable instruments:
- Acquisition of citizenship
- Certificate of legal capacity to contract marriage
- Option to elect Philippine citizenship
- Partition and distribution of properties of spouses and delivery of the children’s Legitimate; and Delivery of the children’s presumptive legitimate
- Repatriation document with oath of allegiance
- Voluntary emancipation of minor
- Waiver of rights/interests of absolute community of property
- AUSF
- Other registrable legal instruments.

SUPPLEMENTAL REPORT FOR BIRTH, DEATH AND MARRIAGE
- About The Service:
A supplemental report using the appropriate form (COLB, COD, COM or Cert. of Fetal Death) maybe filed to supply information inadvertently omitted when the document was registered. Only one supplemental report for not more than two omitted information in any registered event shall be accepted.
